Alameda Point Collaborative (APC) creates permanent supportive housing communities for families who are homeless in Alameda County. Their programs empower residents to lead independent, purposeful and productive lives. The APC commissioned our team to develop a new medically-oriented health and wellness campus, which will be comprised of 90 senior supportive homes, a 50-bed medical respite program, on-site primary care clinic, and a client resource center.
